Teamsters Enrollment Form

In order for you to be approved for either of the Teamsters programs  through SHC you need to have your PCP fill out a form to verify your BMI and/or co-morbidities that qualify you.

• Weight Management Program: A BMI of 30 or greater qualifies you for the Weight Management Program.
• Surgical Wrap-Around Program: A BMI of 40 or greater qualifies you for the Surgical Wrap Around program.
